HCF is committed to best practice privacy protection and only collects information that is relevant to the insurance or services you ask for.

Any personal information we collect is used to provide you with further health services, for instance to develop programs that are intended for the benefit of members. In some instances, participation in these programs is offered to all members. In other instances, the invitation to participate is targeted at specific members who are most likely to benefit from participation in the programs.

Participation in any program offered by HCF is completely voluntary. If you choose to participate in a program then, at a later date change your mind, you are free to withdraw. If you do withdraw from a program it will not affect your member benefits or premiums or your client relationship in any way.

The programs that we develop for the benefit of members are frequently administered by third party service providers. The Healthy Weight for Life Program is being administered by a third party provider called Prima Health Solutions (incorporating Pharmacy Health Solutions Pty Ltd - PHS).

If you choose to participate in the Healthy Weight For Life Program then PHS will need to collect personal information from you to run the program and to assess its effectiveness.

The information that will be collected by PHS includes your:

• name, mailing address, e-mail, contact phone numbers
• height
• weight and waist circumference measurements over time*
• blood pressure, HbA1c (haemoglobin A 1 C – a measure of medium term diabetes control), lipid profile (total cholesterol, LDL and HDL cholesterol), triglyceride levels*
• health questionnaire results (initial and final)

PHS is under contractual obligation:

• Not to disclose your personal information.
• To use your information solely for the purpose of running the program.
• Not to use the information collected to offer other services to HCF members.
• To destroy all collected personal and confidential information at the end of the program following recognised and professional guidelines.

NO INFORMATION WILL BE PASSED FROM PRIMA HEALTH SOLUTIONS TO HCF THAT WILL IDENTIFY YOUR SENSITIVE HEALTH INFORMATION UNLESS YOU EXPRESSLY ASK FOR IT TO BE SUPPLIED TO THEM. ALL YOUR DATA WILL BE HELD BY PRIMA HEALTH SOLUTIONS IN A PROTECTED AND SECURE DATABASE.
