Heart Program

The Healthy Weight For Life Heart program is designed to help overweight HCF and Teachers Health Fund members with or at risk of heart disease lose weight, improve their health and learn the lifestyle skills that will help them stay in control of their weight for life.
It is run by Prima Health Solutions (PRIMA) for and on behalf of HCF and Teachers Health Fund. Provided you are eligible, there is no charge to do the program. All the material you need is delivered directly to you.

How Does the Program Work?

The program is a unique step by step treatment process that is delivered in 3 structured phases over 18 weeks. Each phase has its own unique:

>>>  Eating plan
>>>  Activity plan
>>>  Online coaching plan
>>>  Personalised online self monitoring and tracking
>>>  Personalised telephone motivation, support and advice

Who Can Participate?

The program is available FREE to eligible HCF and Teachers Health Fund members aged 18+ who:

• have a body mass index (BMI) ≥30 AND

• are being treated for, or have a history of heart disease OR are referred by their health fund nurse or health coach

• have a hospital product (policy) that has been held for at least 12 months

• obtain approval from their medical practitioner to participate and maintain supervision for existing conditions

• are willing to provide their progress data during the program

• have NOT previously participated in any Healthy Weight for Life program offered by HCF or Teachers Health Fund

HCF Members - Graduate Claim Forms

HCF members who complete the full 18 week Healthy Weight For Life program may now claim a rebate under their ancillary cover for purchasing additional KicStart™ VLCD to help with their continued and ongoing weight management. Download your claim form from the Resources tab inside the Member's area.

HCF Member - Not Eligible for a Fully Funded Program?

Under the HCF Health Management Program initiative, members who are not eligible for the fully funded program can claim a rebate on a self-funded Healthy Weight For Life© program if they have a BMI of ≥ 30 kg/m² OR BMI of ≥ 27 kg/m² and risk factors such as hypertension, dyslipidaemia, cardiovascular disease, diabetes, smoking
and waist circumference outside target limits for ethnicity.
