HCF Healthy Weight For Life - HEART ProgramThe Healthy Weight For Life Heart program is designed to help overweight HCF members with or at risk of heart disease lose weight, improve their health and learn the lifestyle skills that will help them stay in control of their weight for life.
It is run by Prima Health Solutions (PHS) for and on behalf of HCF and Manchester Unity.
Provided you are eligible, there is no charge to do the program and all the material you need is delivered directly to you.

How Does the Program Work?
The Healthy Weight For Life Heart program is a unique step by step treatment process that is delivered in 3 structured phases over 18 weeks. Each phase has its own unique:
>>> Eating plan
>>> Activity plan
>>> Online coaching plan
>>> Personalised online self monitoring and tracking
>>> Personalised telephone motivation, support and advice
The aim of the program is to help you lose weight, improve your health AND learn lifestyle skills that will help you control your weight for life.
Who is Eligible?
The Healthy Weight For Life Heart program is available FREE to eligible HCF and Manchester Unity members aged 18+ who:
>>> are referred by their My Health Guardian nurse or health coach OR
>>> are being treated for, or have a history of heart disease AND
>>> have a body mass index (BMI) ≥30 (Check your BMI here)
Plus have NOT previously participated in any Healthy Weight for Life program offered by HCF or Manchester Unity and:
>>> have an HCF or MU hospital product (held for at least 12 months)
>>> obtain approval from their medical practitioner to participate
>>> maintain medical supervision for existing medical conditions
>>> are willing to provide their progress data during the program

Healthy Weight for Life Graduate Claim Forms
Graduates of the full Healthy Weight For Life program may now claim a rebate under their ancillary cover for purchasing additional KicStart™ VLCD to help with their continued and ongoing weight management.

Not Eligible for a Fully Funded Program?
Under the HCF Group's Health Management Program initiative, members who are not eligible for the fully funded program can claim a rebate on a self-funded Healthy Weight For Life© program if they have a BMI of ≥ 30 kg/m2 OR BMI of ≥ 27 kg/m2 and risk factors such as hypertension, dyslipidaemia, cardiovascular disease, diabetes, smoking
and waist circumference outside target limits for ethnicity.
